Port forwarding help please ;)?

Hey so on the game i play which is Call of Duty Black ops its says if i am experiencing any problems i should try port forwarding. So now i got the TCP and UDP numbers but i need your help on this. On my routers settings i went to portforwarding and this is what i see https://iihelp.iinet.net.au/support/iiattachlink?id=1658 and this is the information i have to enter now how do i put it in PlayStation 3 - TCP [80, 81, 443, 3074, 5223] UDP [3074, 3075, 3478] thanks
